Oh dear goddess, I recently heard the term "dirty estrogens." It came from a patient asking for my help in ridding her body of said, "dirty estrogens." I googled it and found a bazillion web sites referencing "dirty estrogens" and how to get rid of them (buy something!), but no clear or consistent explanation of what a "dirty estrogen" is. In general, these hucksters seem to be referring to estrone and estradiol as "dirty." But they like estriol, "which is the protective estrogen.” Can you give us a little more clarity about estrogens and estrogen metabolites?
Could you please explain which form of vaginal estrogen therapy is more effective for improving the tissues of the vagina as well as for the clitoris and urethra. I live in Canada and estradiol is only available to us as a Vagifem tablet or contained within Premarin cream. Otherwise Estrone cream (Estragyne) is available. Unfortunately estradiol cream as in the US available Estrace ( and generic) is not available to Canadians—I wish we had the option. A physician told me they equally effective however my understanding is that estrone is weaker. Could you explain this?

As someone who spends a lot of time digging into the science of menopause to try to get to the truth (I feel like Tom Cruise in A Few Good Men), I so appreciate you taking the time to provide this lesson in chemistry. I love the Halloween candy analogy! If you can explain the different chemical characteristics of CEEs versus estradiol in terms of interaction with beta and alpha receptors and SERM-like behaviours, that would be a post maybe only I would like. But I'd really really like it!!
